How do I submit an Institution Validation Form to the school?

The Institution Validation Form is included in the Claims Packet provided when initiating a claim. This form must be presented to the school for completion.
Once received at the designated email, the school will complete the document and return it to the student.
For Arizona State University-Tempe, students are required to send their Institution Validation Forms to StudentClaimInfoRequest@asu.edu, not to the Dean or Business Office as indicated on the form.
