Arizona State University- How do I submit an Institution Validation Form to the school?

The Institution Validation Form is included in the Claims Packet provided when initiating a claim. This form must be presented to the school for completion.
Arizona State University-Tempe students are required to send their Institution Validation Forms to StudentClaimInfoRequest@asu.edu, not to the Dean or Business Office as indicated on the form.
Once received at the designated email, the school will complete the document and return it to the student.
